Vascularization question

Patient returns after his fistulogram. We discussed revision of the fistula to allow maturation. No changes on exam. He is agreeable and we will make arrangements for patch angioplasty to this nonicharia and ligation of the side branch.

I have an idea that I'm not even close on this one but any suggestions would be helpful!  You all usually come through and I depending on you all!!!
