Subject: Vascular surgery help needed--AV fistula involved
Hi,
I've got a vascular surgery that I need help on. Most of the things I'm pretty sure of, but would like a seconding ear. TIA
Right wrist "rest"--?? assuming AV fistula.
ANESTHESIA: @@Local with monitored anesthesia s/l--montra--garis bolus 20 cc.
extremity was prepped and draped in a Pfannenstiel fashion. The patient was given a low grade--?? of Ancef
The artery was then carefully dissected out proximally and distally and little side--?? branches were clipped to help obtain hemostasis ofr the anastomosis. The
A pot--?? scissor was then used to form a hood. It had been marked with a marking pen prior to division
Markable--?? dogs were used to obtain control of the artery proximally and distally.
Th vein was sewn to the arteries using a 6-0 pulling suture in a circumferential--?? fashuion.
The wound was enclosed in layers using a 3-0 Vicryl suture in the subcutaneous layer in a running fashion and a 4-0 Vicryl running subventricular--?? closure.
The patient was brought out and monitored in anesthesia care--?? and taken to the recovery room in stable condition.
Sponge, needle, and instrument counts were reported to be correct at the end of the case--??.

as far as post-end versus postend.....hyphens are used for clarity and in this case I would have used the hyphen.....because it is usually end-stage renal disease....I'd give 'em post-end
If your account is verbatim and they said AV fistula give them what they said. If you are allowed to expand, you could give them arteriovenous fistula.....
HTH (hope that helps)..........

I've heard of psychiatric "postconviction" services or "pretrial" services...such as, "postconviction psychiatric evaluation by a forensic psychiatrist is a new factor warranting sentence modification"...perhaps that's what they meant (IF the patient is a prisoner/arrested, admitted involuntarily, etc.)
