She has no contraindications. The risks and benefits of the procedure have been explained to her. She has not had any ??VC anticoagulants??

"Recent anticoagulants" makes sense
Subject: "Recent anticoagulants" makes sense
Surgeons are REQUIRED to check the coagulation status of the patient prior to surgery so that the patient does not have any complications such as uncontrollable hemorrhage during the surgery. The surgeon probably was trying to make it into a heading, like this:
"Recent anticoagulants: She has not had any."
OR if you heard a definite "D/C" there, she probably could be talking to herself. If you have the freedom to edit, make it into a sentence that fits, like this:
"She has not had any recent anticoagulants."
OR type what you heard, like this:
"D/C anticoagulants; she has not had any." That still is okay and would make sense to another physician.
