could it be trocar injury of the colon, possibly caused by a puncture during surgery?

I'm used to them saying 5-mm trocar or 12-mm trocar.
At this hospital the docs keep saying "Under direct vision, a 'five twelve -mm trocar' and sheath was advanced into the abdominal cavity."
It's singular. Whey are they using 2 numbers, and what is it they want?

Interesting presentation of zonular dehiscence from the blunt trauma with prolapse of vitreous subsequently addressed by Dr. ******* but has progressed with a variant of an "Irvine gas", marked swelling of the optic nerve as well as macular pucker.
He says Irvine gas very clearly, but I've never see this in that context. Does this look right?

The patient states on initial evaluation that he is currently not having any throat pain nor is he stridorous. The patient does have a medical history of a s/l TC approximately a year ago for which he was intubated and actually on extubation had to be re-intubated and had some tracheal scarring
