Don't forget that physicians use anatomic right and left. The chief complaint (which is usually a direct quote from the patient, but obviously not here) would relate to the patient's left or right. However, when the physician looks at the patient, it is opposite, so left is right. It's very confusing. But because this is in chief complaint (patient may be saying "my left") and doc is facing the patient it is anatomical right. Best always to flag regardless. Good question for this board!!

correct - if an MD spells anything, RED FLAG...SM
Subject: correct - if an MD spells anything, RED FLAG...SM
if an MD/dictator spells ANYTHING, it's a RED FLAG to look that spelling up because 95 percent of the time they misspelled whatever they were trying to spell. MDs were not great in grammar and/or spelling (for the most part) - they were great in other areas. *lol*
Good stretch! I believe you are correct. thanks!

NWMNMT:Good grief, good grief, good grief..If this is the case, the doctor should also explain
Subject: NWMNMT:Good grief, good grief, good grief..If this is the case, the doctor should also explain
the meaning in an ADDENDUM.
How do you know what the doctor meant? GOOD GRIEF, are you the know-it-all (hyphens must be put here, as it is used as a noun!) on this board? Get over yourself!
Who understands what it means without an explanation?
The MOST obvious is apparently like I understood it:
5 packs in 1 year.
Maybe the doctor really meant this, 5 packs per year, so essentially a nonsmoker.
GOOD GRIEF, get over yourself!

I have a name that I can't understand in the document anyway (person's last name) and I can't find it to verify spelling - so it's got to go to QA anyway, I come back here and post if/when QA gets it in case someoen else ever has it. TY for those that tried looking also.

Doc may have meant take 2 tablets in the morning and said the q just as a habit. I would certainly question having to take Lexapro at 2 a.m. In my opinion, don't guess - flag it.

Seems obvious he meant ACL.
I have a doc who says stuff like that all the time...give a list of "this is normal, that is normal" only he says the same thing both times. I don't change it, I just flag it.

During CBC doctor says "counts of 7.7" Later in the sentence he says white count of 11,900. Would he be referring to RBC and should this be flagged for clarity????? It's unclear to me but then everything he says is. Thanks for your help.

When I got to the exam, she only weighs 111 lbs, so that makes me even more concerned that he just got lazy and just said "5" because he definitely didn't say 0.5 or even .5, it was just 5. I hesitate on questioning things that could be borderline, I always worry about them getting huffy if I dare to suggest they might have said something wrong, especially if what they said was what they meant, but then I think, I work at home in another state and wouldn't know him from a hole in the ground, what's he going to do, come to my house?!?!?

dictating a medicine of Xanax 20 mg t.i.d. I'm only an MT, not a pharmacist, but 20 mg sounds awfully high, especially when the doses are usually in the 0.5 to 2 mg range. I checked and it comes in 0.25 mg up to 2 mg. I'm just wondering if I should put a QA marker here and ask them to double check or if I should just type what I hear and let it go through. Has anyone else heard of somebody being on this high of a dose of Xanax before? Thanks!

Physical examination of the left shoulder reveals active forward elevation to approximately 90 degrees. Actively she could elevate it to approximately 130 degrees with pain.
He uses "active" twice. Isn't that wrong?
