I worked in Family Practice for a while and they commonly recommended patient's place a few drops of Colace in their ears prior to them coming in to have it, along with the ear wax, irrigated out with a regular ol' Waterpik (ya know, for yer teeth!) of all things! Family practice is known for winging things a lot!

I always thought q. daily was wrong, but have received back corrections from QA with q. daily after I had changed it to daily. This is not a verbatim account. Am I wrong?

This is in regards to treatment of a woman with breast cancer. I cannot decipher if the doctor is saying tamoxifen AFA 81 mg daily or something else. According to my references the usual dose of tamoxifen is 25 mg daily so possibly the doctor is saying "due to tamoxifen AFA but then what medication would sound like AFA? Please help!
