It always used to be tendonitis, now the change.

And you say extend/extension so why not distend/distension?

When I first started transcribing again after a hiatus of about 15 years, I had to put "distension" in my AutoCorrect so that it would automatically change it to "distention" until I could get used to it.

I hate it when the doctors/PAs prounounce diabetes MELLitus as diabetes mellITUS like it's some kind of inflammation or something.

From Dorland's.com:

diabetes mellitus (di″ә-beītēs melīĭ-tәs)

Good grief - you're a highly educated person.  You should know how to pronounce the medical terminology and medications.  Sorry.  Just tired and crabby todayd.

There's an ENT doc who is about to drive me insane! I can't find just one word.
Subject: There's an ENT doc who is about to drive me insane! I can't find just one word.

I've wasted about an hour today looking for this one word the dictator uses over and over. I'll copy the sentence:

"Larynx and pharynx examination reveals normal adenoids, ____________, eustachian tubes, pharyngeal walls, piriform sinuses, epiglottis, false and true cords immobility to mirror and direct laryngoscopy."

The missing word sounds like "coin-ee." I've pulled old reports and everyone has left it out. Also can't tell for sure that he's saying immobility. Doesn't seem like immobility of the vocal cords would be desirable. Shouldn't it be mobility?

Somebody out there must be better at ENT than I.

Pes plano valgus with gastroc constracture, forefoot varus, left side, is correct. Technically, in OP report headings, I was always taught to spell things out like "gsstrocnemius" even if the doc abbreviates because it is a main heading, but it also goes with your account. Hope I have helped!!

Subject: this is crazy!

There were three other places in the dictation that I could not understand what he was saying.

After I deciphered the 'alert and oriented' (with your gracious help), I went back to the other three. Bingo. It was like a mumble-jumble-decipher puzzle piece placed itself in my head, and I figured out the other three.

I love puzzles.

Thanks again!

The actual pronunciation of that word IS "day-breed-MON". I about died when I first heard that. I guess it's French. It sounds so hoity-toity to me. LOL

Gee, ya think? You should see some of the stuff

I don't know who they got working there sometimes.  So much conflicting stuff it's unreal.  I call them on it every time though and send it to my super.   It's hard enough without the conflicting feedback.

Example:

Dictator said HPI (for a heading), so I typed it HISTORY OF PRESENT ILLNESS.  I get QA feedback that says, "If the dictator dictates the header as HPI, type it in as HPI."  So I'm like okey dokey.Next time I get HPI I do just that.  Then I get a nasty note.  NEVER, EVER abbreviate in the headers, always spell it out.  I'm  like geeesh!

One told me to type out 2:00 O'clock (for position, i.e., cornea).

crazy number question
Subject: crazy number question

The company I work for does not want us to use numbers and numbers should never be used at the beginning of a sentence, but does anyone know the AAMT-true style that this should be transcribed?  I know this is a crazy question, but for some reason, I can't seem to categorize it.

Example:

"School grade: 5th.

Members in family: 5. "     OR

"School Grade:  Fifth.

Members in family:  Five."

I am thinking the second, but then again, I am not thinking.   :-)
