Google "* propionate" and cutaneous T cell lymphoma and you'll get lots of hits...I wracked my brain trying to work on your first S/L; I was SURE it was some kind of propionate, but couldn't make the *V* work...now with your added info I'm almost sure it's probably FP.

I would probably make just a minor adjustment as I think he's talking about edema in both ankles:
EXTREMITIES: There is edema of the right ankle at the site of fracture times two, trace to 1+ on the left. Significantly diminished pulses, equal 0-1 DP and PT lower extremities bilaterally. Normal upper.
As for the rest....looks okay to me...not sure about the "equal" part - sorry! Good job!

Conus medullaris and the cauda equina are within the normal limits. Intervertebral disk at L4-5 has mildly diminished signal, indicating desiccation at its interspace.
FINDINGS: There is a mild bulge centrally with facet osteoarthropathy but no significant canal stenosis.
**I'm not 100% without actually hearing the dictation that interspace and facet osteoarthropathy are definite, but with years of rad experience that's the best guess I can make for ya, both would be acceptable in that context as you have listed it. But I at least FINALLY figured out blank #1 (bulge..not bone)!! Yipee! LOL Hope this helps ya!

Most of the ortho doctors I type for say medial and lateral joint line (or lines). I type what they say - if they say line, that's what they get, even if it seems like it should be lines. Same with medial and lateral versus mediolateral. I figure if I type what they say, I'm in the clear. With your example, if it sounds like he's saying medial lateral, I would type mediolateral, since it's the combining form for medial lateral.
As far as the lab, I would type it with the laboratory data. If there is no heading, I space it down one just to make is separate.

She had a trauma and then on her x-rays it looked like she may have an intermoody displaced lateral tibial plateau fracture as she was having pain bearing weight. Given that, I sent her for a CAT scan to evaluate it. She returns today saying she has been compliant, been nonweightbearing, and been in a knee immobilizer. She is now about two weeks out from the injury. Review of her CAT scan today shows that she does have a slight depression of the lateral tibial plateau.
