If I'm not 'speaking' clearly it is because I've had to listen to this mush mouth, speed talker for the past 8 hours and am only about 3/4 done with my quota. I've never had so much trouble... even my ESL doctor makes better sense than this guy.

And I'll ask again, why isn't her QA helping her in the way they want her trained or giving her links to document her answers so she can research. One learns better when one does for themselves rather than being fed by the silver spoon.

Last line of the report, dictating physician says "I will page Dr. so-and-so and discuss with him." The title "Dr." was inadvertantly left out (mybad), admittedly an error from typing too fast and didn't catch it. QA pulled this report and marked it as a "critical error affecting document integrity." I appealed the ruling arguing that it should be considered merely as a typo as document integrity as it pertains to accurate documentation of patient health care (which is what these documents are for) and proper transcription style had not been affected in any way. QA board upheld the ruling implying in their reasoning that the doctor could be offended, i.e., his ego could be bruised, by failing to transcribe the title he worked so hard to achieve.

affect is a verb and effect is a noun. There are only 2 exceptions - when affect is used in psych reports referring to their general attitude and when effect is used as a verb meaning "to bring about" as in "to effect a change in someone". Try to keep it simple.

You made an assumption that I made my comment based on my "moral compass." You are the one being presumptive. As it so happens, I became an MT because the job I used to have did not allow me to go back to school. I recently got my second bachelor's in psychology, graduating summa cum laude I might add, and have been accepted to grad school for school psychology at one of the most competitive schools in the state. So I beg your pardon but the information I gave is not my moral compass. It comes from the extensive education I have received. In addition, I have experience working with kids with developmental delays and my best friend growing up had autism. I assure you if a doctor used that word with some of my friends who have children with disabilities, not only would the doctor get an earful so would the AMA.

Interestingly the PET scan revealed minimal FDG uptake in the right thyroid bed consistent with the right anterior tracheal lesion, and also minimal uptake in what is believed to be persistent thymic tissue in the "intra-media-stynum", but nowhere else is it noted. MRI was reviewed as well. This confirms a 1.4 cm enlarged lymph node on the right side at the level of C3......

What is the basis for any jealousy here? That concept baffles me. You obviously spent a lot of time into the events of today and I would suggest an alternate outlet...like a l-i-f-e. I do not always come here to flame or denigrate others. That you consider me trouble is of no consequence to me. I have no concern for how my posts are perceived and when I post, I'm usually my ADD and dyslexic self with multiple lines of thinking bouncing around in my head at the same time. I do not self check my grammar or spelling. I'm happy with my QA score of greater than 99.5% monthly because when I work, I allow no distractions. Before I was out of high school, I had the skill set I needed that today's poster lacked and that you commented on as a general rule for all newbies.
If you do not understand my point, that is your failing, not mine. Others have repeated my point so I hope it is a bit more clear to you by now. I will not "leave this newbie" alone (not that I now have a vendetta against her/him now) but you hold no power over my actions, just as I have none over yours. I feel no threat from you, do not feel diminished by you and feel sorry that you are so worked up over today's events.
I had no one to ask questions of at all when I entered MT as I came in as the MT supervisor with years of experience in being a records auditor. When I went home and entered the acute care arena, I had more books than I could count, with my favorite being "A Syllabus for the Surgeon's Secretary" (first published in the 50s). I didn't make a lot of money my first year at home but every report was superior to those of the other MTs on the account. I feel today's newbie has no excuse for not being able to find many of the answers to many of the issues that were addressed today other than by posting entire reports for strangers who may only think they are qualified to answer. Asking for QA help on entire reports is a crap shoot. On a forum where no one knows who anyone really is in real life, you can easily get crappy help anot not know the difference. Your mileage may vary.
I hope you find some peace and a healthy outlet for your frustrations.

how the client prefers. If you are being marked down for not putting them, then I would go ahead and add them if you cannot get an instructor to let you know one way or the other. Sometimes the docs do dictate vital signs separate from the PE, and unless you have been told to move them under the PE heading, then I would leave them as dictated.
