This is a 4-year-old with metabolic-type cardiomyopathy who required "Amikin chest tube treatment".
I see that Amikin is an antibiotic, but do not feel comfortable with the references I have found with chest tube and/or cardiomyopathy. I would appreciate any opinions on this. Thanks!

'and' would not stand on it's own as a sentence and therefore does not need a comma.

In med list, doc states Carbetrol 900 mg q. h.s.
There is no mention of seizures, but there is mention of back pain and I noticed that Carbetrol can come in 300 mg tablets, and that it can also be used for trigeminal neuralgia, which there is no mention of either. This is an ESL, so it is posisble that I am not hearing right, but it was never mentioned again. Past medical history is bipolar, dyslipidemia, hypertension, and gastritis. Thank you!

I type verbatim, so if the doc dictates of I put it that way, if not, I don't, but have seen QA do this to me many times. To me this is so insignificant and it does not critically change the patient's care in any way, I just let it go. Some of my QA people change it, some don't, wish they'd be consistent, but I really don't worry about it.

But I would think that the sentence would probably be:
"Heart size and epsilons are normal."
Can you hear the "and" if you listen real carefully?
Epsilons are not related to the heart size per se. They're waves that can be seen on an EKG.

I was taught this based on the BOS.
If 7 cm is being used as an adjective (as in describing something or modifying the noun), then it has a hyphen. Otherwise, it does not.
Examples:
1. The patient had a 7-cm area of rash on his arm.
2. The rash was measured at 7 cm.
3. A 7-cm Batman figurine was removed from the child's ear.
4. The Robin figurine was only 5 cm tall.
In examples 1 and 3, the measurement is being used to modify the nouns and help describe the noun further. I hope this helps. :)

Your answer might be dependent on what type of depression is being evaluated.

If your are doing a diagnosis as a heading, you can recast the wording
DIAGNOSIS
1. Mass, 3 cm.
2. Other stuff.
BOS discourages the use of a number at the beginning.
Advice is free and should be verifed with your QA staff for how they want you to do it.

I need opinion on something from AAMT regarding style...
I know the latest from AAMT regarding using numbers is to use 2, 3, 4, etc instead of two, three, four in the body of a report. I just think it looks tacky to say "on postoperative day 2" or "after 3 days", etc. I hate doing it. It's not so bad when referring to meds or technical stuff. Does everyone follow this AAMT "law" now? Just wondering what everyone else does???
In my opinion, it should be CIWA and the doc is just saying "S" when he means "C".
