Hi and thanks for responding. I am now in the impression and it sounds like he is saying *status post AP resection of a rectal carcinoma* - perhaps that is what he was saying at the outset and not A to E. He is definitely NOT saying *end-to-end* but thanks for trying!!!

I am pretty sure she is saying "shelfing" (yes, with an "f"), which she says twice, and then refers to the "skin shelf". Here is how she says it:
"Over the central aspect of the wound there is shelving of the superior aspect of the skin. The wound was cleansed with normal saline. Iodoform gauze was placed under the shelving and Steri-Strips were reapplied.
A: Postop wound infection.
P: Keflex 500 mg p.o. q.i.d. Potential side effects reviewed. The iodoform without Betadine was placed under the skin shelf."
I guess we're talking rolls of fat (if so, I've got several shelves myself . . . LOL)

This could very well be my problem. I'm using my third Microsoft Ergo keyboard and, come to think of it, there's the built in "hump" below the keyboard, and that is what is hurting my trapezium area; I think I am banging that area against that built in "hump". I've never had this pain before, but I do have a new desk where I have a very small keyboard tray, so it may be the way I'm positioned. I'll try to move the keyboard to the right a bit (and put the mouse on the top part of the desk instead of squeezed with the keyboard).
I can tell a little difference this soon! I'm not banging that part of my hand on the hump anymore!
I'm also thinking this might also prevent me from closing the Word program so much. Sometimes I hit a couple of keys, and all of a sudden Word closes on me. I'll bet this keyboard is shifted too far left, and I hit Alt C or something like that!
We have probably solved my problem! Thanks so much.

This dictator normally says, "C&S clear." The sentence that I posted above is the one time that I have had him dictate anything else. He is pretty clear.
I appreciate both of your responses, but just wanting to make sure it is C&S rather than CNS (like you said, central nervous system came to mind for me, and I also had thought of culture and sensitivity . . . just not sure).

We normally list them pretty much the same way that you do, but I just had not had one say, "AXIS IV: one, two three." before . . . I think what she meant was it was the same info as listed in AXIS I, II, and III because I saw an old report of hers where it was listed as, "AXIS IV: AS ABOVE." So, I wound up using Roman numerals and just mentioned it to the boss in an e-mail. She's still looking over my stuff before sending it to the client, so I'm sure she'll let me know.
