If the hyphen is used to modify, I am hoping that this is correct. Please excuse my lack of ability to absorb this. It seems like my entire report will be filled with hyphens as many nodes are seen...it just looks too busy to me to be acceptable and having a hyphen in one place to describe the same node should be the same as when the node is described at the end of a sentence...I should lobby for changes, huh?

I have never done an op note and have certainly never done a groin node dissection! There are no sample reports available. I have done this doctor before in his clinical setting and he is one of my favorites, but no op notes - the problem is a very poor quality recording - and in an area that I do not have experience!! HELP!! Each line is a separate issue. Some I just need some verifcation that I did it right.
We dissected the lymph node bearing tissue off of the external __s/l dish__ muscle, exposed the femoral nerve artery and vein.
We used hemoclips and silk ___ for hemostasis and controlled lymphatic vessels.
We dissected along the superficial femoral artery and femoral nerve to __revolve?__ the lymph node bearing tissue from this area.
We irrigated __and assured?__ hemostasis
We identified the ureter which was reflected medially.
assured hemostasis placing __ FloSeal __ along the obturator foramen area and there was no sign of bleeding
We closed the fascia with a running loop #1 _PDS__ suture.
closing the femoral canal defect with an interrupted, figure of 8, Ethibond suture.
