I have been out of the loop for a while and need help with the usual verbiage used in a review of systems.  Where can I go to find it?  Please help.  Thanks

This is a laparoscopic bilateral tubal ligation with Falope rings, a transobturator suburethral sling, and a cystocopy. (whew!) 

'An infraumbilical incision was made with the scalpel.  While elevating the anterior abdominal wall with _____ clips, the Veress needle with the CO₂ gas _____ was advanced into the peritoneal cavity.  Intraperitoneal placement was verified by a drop in pressure to 0 mmHg.'

1st blank sounds like -- Howell clips or Hallow clips?

2nd blank sounds like -- floam?

Any ideas?  This new account is killin' me! 

I e-mailed AAMT with this same question about a month ago.  Their response to me was as follows:

"He may be dictating "hour of sleep" because he expects you to use the abbreviation h.s. However, this particular abbreviation is on the Dangerous Abbreviations List, and we are encouraged to transcribe this as either "nightly" or "at bedtime." If the dictator questions you about this, you can show him the appendix in your Book of Style or refer him to the Dangerous Abbreviations List under Patient Safety Goals on the JCAHO web site (www.jcaho.org)."

I did give him a sample of _____ (s/l rosarum) 8 mg one right before bedtime, discussed side effects.

Unfortunately, as (s/l):THEY GOT WENT UP in CPAP, all the way up to 13, it was an inappropriate level and they could not get higher.  The patient continuously asked if they could decrease the pressure for comfort.

Go to Tools, at the bottom of the meun there is a customize button, click it. Once you are there open the Command tab.  You will see a list of categories on the left and commands on the right.  Click on the format category and you will see all of the format buttons you can add to your tool bar...  about 1/2 way down you will see the superscript and subscript buttons...  Click on the superscript and drag it to your toolbar.  I usually try to put it with like buttons, like the bold, underline and such.  The button should insert into your toolbar and now when you need to superscript a number you can just click on that button before you type it.  you do have to remember to click the button off after you have typed the superscripted character.
