It is still x2 because it's not an abbreviation.
Times two or times 2 would be corrected to x2 at my company in any portion of the report. However, if the MT has changed "times" to "for" (D: Patient with lesions x2 weeks. T: Patient with lesions for 2 weeks.) it would be left as that is acceptable as long as it is not a verbatim account. This is how it is where I work and if your company and follows BOS guidelines. That being said, I don't always agree with the BOS but I do agree with this issue of "x" versus "times."
