I'm doing an H&P on an obese patient with facial suffusion and a 2-week history of fatigue, 15-pound weight gain, peripheral edema, dyspnea, dizziness, and tingling.
In the assessment, he says historical and physical exam findings consistent with s/l tic leukian or tic loo can syndrome. I can't quite figure this out, and my spelling must be too far off to catch a clue. Can anyone point me in the right direction?

Is dysmetabolic syndrome and metabolic syndrome used interchangeably? In this dictation the doctor refers to it in the past medical history as metabolic syndrome and then in the assesment/plan as dysmetabolic syndrome.

I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN. Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading. Hope that helps!
