"Do not use commas to separate drug names from doses and instructions. In a series of drugs for each of which the dose and/or instructions are given, use commas to separate each complete entry. Exception: Use semicolons or periods when entries in the series have internal commas. Medications may also be listed vertically.

The patient was discharged on Coumadin 10 mg daily.
The patient was discharged on Carafate 1 g four times daily, 40 minutes after meals and at bedtime; bethanechol 25 mg p.o. q.i.d.; and Reglan 5 mg at bedtime on a trial basis.
He was sent home on Biaxin 500 mg b.i.d., Atrovent inhaler 2 puffs q.i.d., and Altace 5 mg daily.
CURRENT MEDICATIONS
1.Lescol 2 mg at bedtime.
2. DiaBeta 5 mg 1 q.a.m.
3.Aspirin 325 mg 1 daily.
