I have been typing: Deep tendon reflexes +2/+2. Does that seem correct?
For half plus tenderness of abdomen, I have been typing: Abdomen with ½+ tenderness. I make sure it changes from 1/2+ to ½+ so that it doesn't look like one over two plus. Does this seem right?
