Per this ESL dictator:
In dx, patient has s/l SIT secondary to right-sided heart failure. (yes, he spelled it out)
Later in report, patient has history of SIT cirrhosis.
His dxe:
Epistaxis, resolved.
Status post respiratory failure.
Status post left thoracoscopy.
Right atrial thrombus per CT scan of the chest.
Status post mitral valve repair.
Status post tricuspid valve repair.
