Question about DC'd

If a doc says DC'd meaning discontinued or discharged, do you type it DC'd, dc'd or do you spell out discontinued or discharged?

Any help would be appreciated...this has been bugging me.  I have one dictator who uses this often and cannot get a clear answer from QA.
