The patient has an asthma-like illness which was aggravated by *poxy* type chemicals. Tried to listen to toxic but couldn't hear it. Any other ideas? Thanks!

Under the HPI, the patient says that after a prolonged plane ride he noticed pain in his calf *leh-foh-LUM*. The assessment later is pulmonary emboli.

"He is on assisted control 500 x 12 x 40%." I actually thought this was "assist-control" but it sounds like the dictator is saying "assisted." And I have no idea how to express the numbers part. If you're good at pulmonology, I'd appreciate the help!

I just can't find this in my resources:
On CT it was a 2.5-cm lobulated density with *s/l glucose aved*, which was read as 70% probability of showing neoplasm.
