Should that be recast, such as patient was admitted as a 302? I've had this doctor about a million times and he has never said 302'd before.

Subject: Psych Report - Hematolimisis ????
Hi - I'm working on some psych reports and I cannot understand a word that seems to come up in every one of her reports. It sounds like she is saying hemotolimisis. The context is as follows: ...denies any history of pneumonia, hepatitis, rheumatic fever, sexually transmitted diseases, hematolimisis (?), melana...
Anyone have any ideas what the word should be??
THANK YOU!!!!

My questions are in two sentences one after the other. Sometimes he says period but then really continues on with the same sentence, so I'm not sure if the first sentence is really a complete sentence or not, so the sentence before basically said a PET scan revealed an increase in the size of the hypermetabolic subcarinal lymph node, consistent with subcarinal metastases.
1. "Stable [s/l lock laid right] pleural fluid thickening, but the associated hypermetabolism has resolved." This could maybe be "stable likely right pleural," but I'm not sure if that makes sense and I think I'm hearing a "d" at the end.
2. "There was a small right level 2 node that has become [s/l non rye valk] consistent with an evolving inflammatory process."

Dictator describes the patient is inconsolably tearful, then says "appears incontinent of affect." I have never heard an affect being termed incontinent. A Google search did not come up with anything using that exact phrase. Does he mean she can't control her affect or mood, like one would not be able to control bladder????
