The pt with stroke...MRA showed...The proximal MCA looks very normal, as does the "sigh fen" as does the carotid bifurcation.

At this point, careful carotid endarterectomy was done. It was begun at the common carotid artery. A wood plane was obtained and it was transected proximally and carried towards the bifurcation.
