Subject: Breast capsular contracture - surgery

Left breast has a significant capsular contracture, more so medially, where there seems to be some tethering of the pectoralis muscle which is displacing the implant laterally.

We may have to performan AlloDerm sling medially in order to reconstruct the ______________ of the breast.

Word was totally cut off. Any ideas?  Thank you

Just curious how I would write the "15 15"  15/15 15:15.  Not too familiar with this.

Help?  The dictator is saying the letters a-b-  and a-d-  prior to pronouncing the rest of the word duction.  Do they just mean abduction and adduction?  This seems simple and easy maybe; must make sure though.  Thanks!!!

abduction vs adduction
Subject: abduction vs adduction

I will try to make this question make sense -

When a doctor dictates A-B-duction or A-D-duction, is he supposed to say 3 letters on one or the other? i.e. ABD-duction or ADD-duction? I'm just wondering if there is a rule that they go by that will help me to better know which one they are trying to say when they are not too clear.
I tend to go with whatever they say first, like

if they start out saying something like *eyes were anicteric* then I go with past tense the rest of the way no matter what. Just easier for me to remember. My account is not verbatim, though.
some docs say ABduction and ADduction
Subject: some docs say ABduction and ADduction

In my dictation, they will dictate A B -duction and A D -duction to separate the words because abduction and adduction sounds so much alike.  Just been my experience.  I have never posted a reply, but thought I could offer some advice on this one.  Hope it helps.
Would tend to do a dictated
Subject: Would tend to do a dictated

All dictators have their own style (they use particular phrases etc) I would tend to give them what they want.  Some may say first diagonal branch, some  may have M1 or M2 or (marginal 1 branch and marginal 2 branch.)

However, if it is a verbatim account it needs to be typed as dictated.

No, just the 2 letters to give you some direction. ABduction or ADduction.

increase the tendency for coagulation/DVT. In this regard, I agree with you.

However, maybe the elevation of protein S at the time of the DVT rules out protein S deficiency as a probable cause of the thrombotic episode. Is there any such implication? The clotting mechanism involves a complex cascade of activation events of numerous factors, and Protein S is just one cofactor for another anticoagulant protein.

Still, it would be wise to flag the doctor and let him put the final word.
