I figured this out because my mom was gravida 4, 2-2-1-2:  First pregnancy resulted in a miscarriage, second full term, third premature twins who did not survive, fourth full term.  So it could be that one of the pregnancies in your example was a multiple birth.  Of course, it's always possible for the doc to screw up the numbers, like when they try to number diagnoses...
