You heard it right. In Orthopedics, you'll come across the procedure of "arthroscopy" quite a bit. The Ortho surgeon makes small incisions called "portals", through one of which he inserts the scope to look inside the joint, and through the others he inserts other instruments as needed for cutting, shaving, biopsies, etc. Once he completes the procedure, he closes those incisions which become the "portal sites" that he is talking about. He obviously is checking during the followup visit to see if there is any infection at the incision sites.

The girl that worked in my office who did transcription before I started used to type port hole instead of portal.  It looked really funny in the transcription.  For some reason, the docs never corrected her though!!!

Can't make out this word.  Sounds like ace-ee-iss with the stress on ace.  Here's the whole sentence.  "A CT of the abdomen with contrast revealed nodular [ace-ee-iss?] entry into the head of the pancreas and the omental fat, which was felt to represent adenopathy or recurrent tumor."  Any ideas?  Thanks!

EKG help - J Point

Cardio doc dictates EKG as follows: 

EKG demonstrates normal sinus rhythm at 61 beats per minute with normal axes, normal intervals, J point elevation 1 and 0; otherwise, no ischemic changes.  


But just not sure on how to type the 1and 0 or if he is saying 0, but sounds for sure like 1.  Anyone with any ideas.  This is a new one on me?  Any help appreciated!!!

Someone new at this might have to look-up a lot things.  Sometimes you just get to the end of your rope with researching especially when the doc isn't sounding too clear or the word is slurred.  We must support one another though not tear each other down.  Why can't people just go by the old saying of "if you do not have something nice to say then do not say anything at all".  To come off like a know it all proves what?  Absolutely nothing in my opinion.  Not everyone is up on everything.

Although he does mention "near the perforation" later in the report.


My point is, docs sometimes use the abbreviations they use in speaking in their reports as well. Most of the time I will type them out, but it bugs me either way - I like to type verbatim and not add anything the doc didn't say, but if he says it wrong, what are you going to do? (Except maybe leave him a message.)
