Thanks Grace. They have me stumped as well. The doctor didn't seem to change his mind at all in this section of the report, and the only other thing he mentions about hypertension is: A history of hypertension with treatment on dyazide daily. In the report the first and second phrases are actually combined, so I've revised them a bit.

1. Past Medical History: A transient ischemic attack, [s/l a critizens that time with aspirin]. Appendectomy in the [s/l 3-D] and [s/l Cs]. Tonsillectomy.

2. Impression: Secondary [s/l dientness] hypertension.

Her PMH is hypertension, CHF, dementia, depression, osteoarthritis, diverticulosis, left hip fracture and she is in the ER for vomiting.  She is hypotensive & saturating 84%.  Her other PSH is hyesterectomy & thyroidectomy.  Other than that, I haven't a clue yet.  Still typing.  The PA is ESL which could be a factor, but I've been typing him for years and he clearly says TUD surgery.

While the gastrocnemius and soleus are 2 separate muscles, they come together in a common insertion to form the Achilles tendon, hence the frequent references to the "gastrocsoleus complex." The term "gastrocsoleus" would not be seen in Grey's because the information deals with the anatomy of each muscle individually, but that does not mean the two cannot be referred to and transcribed in the above manner.

Stedman's shows it as "gastrocnemius-soleus complex" while HPI Orthopedic Words and Phrases makes a reference to "gastroc/soleus." I have typed it as one word for many years and have never seen any QA objection to it. The bottom line will be, if your particular company or account has a preference, then go with that. In the absence of such a stated preference, however, it is acceptable to transcribe "gastrocsoleus" when dictated. Some will view it perhaps as slang or whatever while others will see it as merely constructing a medical term as done within the acceptable parameters of medical terminology for the purpose of referencing anatomical structures that share a point of commonality.

I would do 60%. I have several doctors that will say 60 (as in ) 6-0. Clarifying the numbers so you don't put 16. They do 50, 5-0 so you don't put 15 because they sound close. It just sounds like sixty-six-zero. Put what you feel is right but I would be from experienc it is 60%

Regarding tonsil and adenoid surgery, sentence is as follows:  The table was turned 90 degrees and a shoulder roll and _____  (s/l memorial header?) were placed. A Crowe-Davis mouth gag inserted to retract the patient’s tongue.

I would recommend getting Stedman's Infectious Disease and Organisms book.  It has everything (well..pretty much) listed both by genera and species and shows any subspecies.

means after, so post phlebotomy would mean after a phlebotomy. It could be any time after, 1 hour, 1 day, 1 week, etc. The O in post is a long sound, not short. It could also be past phlebotomy or, as you say, just a false start.

With all of my QA's efforts and mine as well, which included asking my fellow MTs on this board, we have found no such word as "acisional". 


Ahem, it was brought to my attention, however, that this is indeed the way the doc dictated the word 3 (count 'em three) times, and that I was not mistaken in that respect.  (HUH?????  Didn't I say that already??  LOL)


Anyway, look for this newest, doctor-made-up-word .....if the dictator says "acisional", that is what he/she means!  And, it's probably coming to a new Stedman's Ortho/Rehab Words publication in your area sooooonnnnnnnn.........
