The appropriate portal incisions were made on the right knee and (I would use ANTEROMEDIAL here) portal just proximal to the patella were the (could it be ARTHROSCOPIC CANNULA or ARTHROSCOPIC CAMERA?) was inserted



Also involving the posterior horn of the medial meniscus using a basket (I usually hear FORCEPS here. Could that be it? Otherwise it is probably RONGEUR) and 3.5 (should be FULL-RADIUS) shaver.

He was gently injected with 1% lidocaine 2 cc with a tendon sheath type block. He tolerated it well. A pen/pin was placed between the third and long fingers. A closed reduction was performed. Improvement of the reduction was noted although there was still slight deformity.

This patient has knee impact injuries with bruising.


McMurray’s is negative for both medial and lateral compartments.  s/l Dover's maneuver and Lachman maneuver, as well as the medial and lateral collateral ligament testing, are unremarkable.

he says this five times and is so mush-mouthed


The first three times he says it while discussing x-rays and CTs. This shows a compression fracture of L3 with compression of the superior endplate with **Meth-in-fac-late** present within the body, all contained within the body itself. 


Under impression:  History of a compression fracture with treatment of the meth-in-fac-late at the L3 vertebral body.


Under plan: Pedicle screws could not be placed into the pedicles of L3 because of the meth-in-fac-late

The rotator cuff was then repaired back to its insertion site utilizing SUPRAREVI??anchors spectrum SHUTTLE RELAY TRAY? the sutures were passed through the rotator cuff, saved and tied from the lateral portal while viewing from the anterior portal.

OP report lumbar spine, I cannot find this type of "view." He is either saying "and/or toggle view" or and orthoggle view" along those lines. I hear a defintie th sound, though, and cannot find toggle view. 


Each screw at L4 and L5 bilaterally were confirmed with radiographic confirmation and **orthoggle ** views.

Not sure if the long pause is a cut-off or he's just resting. Would "odontoid" fit there?

I'm dealing with a doctor today with SEVERE laryngitis. This is torture.

He is dictating on a patient who sustained an injury to his right fifth finger.

During the exam he states "He has slight s/l FLECK-OR-AB activity when asked to straighten it out."

Can anyone make sense of that?

On physical exam, positive findings are limited to the left 5th finger. She is a straight splint. I removed that splint. I have placed a s/l REDUCTOR/REDUCTIVE force at the fracture and placed her in a tape splint.
