Here's my interpretation of the way your sentence should probably read.  Maybe you'll hear this after you read this. 


 


The patient is a plethoric, tremulous, dyspneic-looking patient with chronic obstructive pulmonary disease or sleep apnea. 


Listen again to the "or sleep apnea" and see if you can hear "and sleep apnea." 


If you really want to learn something, why not look up the definitions of these terms so you can get a good picture about what you're transcribing.  It really makes it waaaaayy more interesting when you can follow along.  It certainly helps with how boring it can get.
