I often see the ASR type "open reduction internal fixation". I was always taught it was "open reduction and internal fixation. They are 2 separate procedures as far as I know. Am In incorrect?

In this op note, the doctor says a "one five recon plate" was trimmed to the appropriate length, and I placed a "one five" screw distally. Would you agree this should be transcribed as 1.5 (as in mm). He didn't say mm, and I won't go so far as to put mm in the report.
