  Weight was 9 pounds and ______ (no idea) at 1 minute and 9 and 5 minutes.

Sounds like "ak-ma-phytin" 1 mg IM given to newborn before discharge, no discharge diagnosis other than healthy newborn.

"Initially there was a little (s/l meck?) and he had some facial bruising and was rather plethoric on initial exam and was monitored every shift with (s/l TCB?).  On the second day he was found to have a 6-ounce weight loss and his (again, s/l TCB?) was 12.2."

On newborn:  She continued to have brief episodes of apnea and bradycardia that was thought to be secondary to reflux. A sleep study with _____ (starts with p and ends with o) was obtained that showed severe gastroesophageal reflux.

It sounds like doc is clearly stating 0.5 mg, but I can't verify in my drug book or google.

small sl-PAFs were noted near the interior chamber haptic positions temporally in the left eye.

this lesion did stain with fluorescein; however, it was noted to be sl-sidal negative.

he also says the lesion is at the 7 o'clock sl-clock R position.

There is contraction across the entire field of vision in both eyes.  Impression is _____ ischemia with field defect, stable.

Please help with the "sind " part. I know it is short for something.

Neurological: s/l just gross and nonfocal.

Maybe nerves gross and nonfocal? Aahhhh!

Best corrected vision with the refraction of plano minus quarter axis 130, 20/20 right eye.  Is this worded correctly or should there be some commas in there??

Also, does this look right? "The patient required a +1 addition add for reading."

Doc is doing a well exam on a child 10 1/2 months old.

States: "Weight, height: S/l No FC ?
