Beta Blockers
Beta blockers have also been studied for the treatment of orthostatic hypotension.9 The three agents most widely studied are propranolol, piûdolol, and xamoterol. Propranolol’s mechanism of action is believed to be antagonism of beta-controlled vasodilator effects of catecholamines. Pindolol is a nonselective beta blocker with intrinsic sympathomimetic activity. Patients with orthostatic hypotension may have reduced sympathomimetic activity, and the agonist properties of pindolol account for its usefulness in these patients. Xamoterol, a partial beta1-selective agonist, has been shown to be useful in patients unable to tolerate pindolol. Because xamaterol is an investigational agent, its usefulness is limited.
