Two questions on lung cancer patient report

My questions are in two sentences one after the other. Sometimes he says period but then really continues on with the same sentence, so I'm not sure if the first sentence is really a complete sentence or not, so the sentence before basically said a PET scan revealed an increase in the size of the hypermetabolic subcarinal lymph node, consistent with subcarinal metastases.
1. "Stable [s/l lock laid right] pleural fluid thickening, but the associated hypermetabolism has resolved." This could maybe be "stable likely right pleural," but I'm not sure if that makes sense and I think I'm hearing a "d" at the end.
2. "There was a small right level 2 node that has become [s/l non rye valk] consistent with an evolving inflammatory process."

The patient was started on a high-fiber, high-fluid diet and her stools are passing easily. She does think that this in combination with the s/l *befalo* is making her feel better on a daily basis.

She has a history of rectal prolapse and had ____water placed and there was some question if the ____water perforated her vagina creating a rectovaginal fistula.
It sounds like he is saying "tertiary" but that does not make sense. I read somewhere that warm water is sometimes used to reduce a prolapse but I'm not coming up with a word for "warm" even in the thesaurus that sounds like that.

MRI of the brain did show a 6-mm area of restricted diffusion involving the midline aspects of the splenium of the corpus callosum, which was representing an acute to subacute infarct process. Was less likely to be considered for a metastatic focus. Right-sided parietal calvarial 7-mm lesions were also noted, suggestive of an s/l inofenocious metastatic focus.

This is exactly how the Dr. dictated it-- The patient is a plethoric, tremulous, dyspneac looking like a patient with chronic obstructive lung disease or sleep apnea.
