I always thought q. daily was wrong, but have received back corrections from QA with q. daily after I had changed it to daily.  This is not a verbatim account.  Am I wrong?

I guess I'm asking if he said it exactly as you put it, "320 daily." That could be taken in different ways, especially if he didn't specifically say mg.

According to my references the usual dose of tamoxifen is 25 mg daily so possibly the doctor is saying "due to tamoxifen AFA but then what medication would sound like AFA?  Please help!

He is actually saying "drop", one drop. He is saying the abbreviation, which is "gtt". If you can use abbreviations, you can use this. Otherwise, just use "drop".
