"The pt works at McDonalds."  If the doctor would have said "The pt works at a McDonald's restaurant," I know to put an apostrophe.  However, do I need an apostrophe or not -- assuming that it is a restaurant but leaving the word restaurant off?  It is not plural.  Thanks.

encounter in my work are highly intelligent professionals who don't abide by your "fast food" rule of speech. If they did, I would not trancribe for them. Your explanation is ridiculous!

anterior chain adenopathy?
Subject: anterior chain adenopathy?

The doc is stating "he has mild anterior chain adenopathy" ( it sounds like that!) but she does not give any headings for her PE --just rattles off comments for each one, so I am wondering where this goes. Any suggestions? What I have found so far may indicate NECK, but am not sure.

Thanks!
