I use SmartType and something similar happened to me regarding the gobbling up and the hour glass going crazy. I ended up getting more computer memory and it quit happening. Also, if Word 2003 is bundled with MicroSoft Works or MicroSoft Office, it will also happen.

capitalize the entire word of the product as FIBERSOURCE but I think it's acceptable to use lower case as FiberSource.

but it depends on if your account is strict "type what they say" and abbreviations like this are okay. I'm not in my office so I can't check BOS, but I'd bet the BOS says write it out unless you have strict rules about typing what is dictated.
That's interesting 'cause the 3 companies I work for still spell out 1-10.

The doctor's really could care less what is written BOS. They are interested in quality work. It is funny how some companies and transcriptionists breath/live it. I am sure there are some doctors out there who are counting every key stroke but most just want to practice medicine. btw....still spacing twice after periods also.....so much for the BOS.....it just doesn't have a whole lot of power these days.
I believe that is only if the company is compliant with JCHO rules. Not all companies are.

departments Lowercase common nouns designating department names; reserve capitals for proper nouns or adjectives, in addresses, or when part of a federal government agency name.
She is head of the St. Mary's Hospital surgery department. He works for the State Department in Washington, DC. The patient is head of the English department at the local state university.
However, capitalize a department name that is referred to as an entity.

divisions Lowercase common nouns naming institutional divisions.

internal units Lowercase common names for internal units of an organization.
The patient's medication was changed because apparently the pharmacy can no longer obtain paregoric.
Exception: Capitalization may be used for such internal units in the entity's references to itself in its own formal and/or legal documents.
Please note the change in Pharmacy hours...
Capitalize internal elements when their names are not generic terms.

I love it when we can "out wit" them. Of course a lot of these guys, not only ESLs, make up the stuff. The first thing they taught was about the prefixes etc in the old transcription classes. I had been typing for about 2 years when they offered the class at a junior college where my husband was attending, he got me the book, which I still have, and I almost keeled over at all the different combos etc.

Tendinitis: Because this word is based on the Latin word tendo, with a genitive singular form of tendinis, and a combining form that is therefore tendin, the spelling tendonitis is irregular.
