This came about in a discussion with another MT. Per the client profiles, what we "normally" did was as you suggest (though I don't know that I've run across the D5, but she got hit with a whole point in Qasar feedback for Protocol/Format). In the time I have been working with MQ, I've never seen it corrected. I left one out - C. diff, and not in a diagnosis. I have a ~very~ verbatim account in which I cannot even correct a dictator who says "interruptive sutures", but I was hit with Critical Feedback in Qasar for typing C. diff in the body of a report, for instance. Same with neuro - each time, .5 points, and same with postop. Some QA people will give .5 points once for the error, and a VERY few will give it each time they see it in the report. (That can add up fast, depending on the dictation!) This dictator specifically said "hypomangesium" instead of hypomagnesemia. Critical Feedback - 2 points. What is the best thing to do in these situations? Go against the client profile in favor of Qasar and AAMT Guidelines? It's confusing.
