Subject: piece of lung biopsy. Does this make any sense? please sm
sentence:
we had to ask for a piece of lung biopsy because he had never been biopsied before.
Probable got mixed up and is supposed to be piece of lung tissue, but maybe I'm wrong. Anyone heard of such a term? Should I just flag it?
Lung disease
Subject: Lung disease
This is exactly how the Dr. dictated it-- The patient is a plethoric, tremulous, dyspneac looking like a patient with chronic obstructive lung disease or sleep apnea.
Did she mean to leave the a out before plethoric? That would make more sense but I know you guys would no better than I. I'm still in school and learning. Thanks for the help!
Please SM re lung disease
Subject: Please SM re lung disease
Sometimes an "uh" sounds very much like "a."
Here is how I would transcribe it: The patient is plethoric, tremulous, and dyspneic, looking like a patient with chronic obstructive lung disease or sleep apnea.

you don't need to add an 's' to "cc" It's "1 cc" and "10 cc" for example. :D

"cc" is a possible future listing in JCAHO "do not use list" sm
Subject: "cc" is a possible future listing in JCAHO "do not use list" sm
JCAHO requires its members to follow an official "do not use list" and "cc" is being considered for possible future inclusion in the JCAHO "do not use list" to be replaced by "mL" because it can be misread as U or unit when poorly WRITTEN...typing it is not as problematic but I go ahead and change it to mL to encourage dictators to make the "mental shift" away from "cc" ...hope that helps! see this link http://en.wikipedia.org/wiki/List_of_medical_abbreviations

The patient swallowed a fentanyl patch. This is the discharge summary and in it, the ESL doctor says, "Transaminitis, most probably secondary to N A L D vs Lortab intoxication." She never says these letters again and all I can document is neonatal adrenoleukodystropy. Since she's almost 60, I don't think that would fit.
