Review of his x-rays today shows that he has some fourth and fifth proximal phalanx fractures of the shaft, just a little oblique in nature. The s/l 'ulnar-peri' was significantly displaced.
Hx: Fourth and fifth phalanx fractures.

In this op note, the doctor says a "one five recon plate" was trimmed to the appropriate length, and I placed a "one five" screw distally. Would you agree this should be transcribed as 1.5 (as in mm). He didn't say mm, and I won't go so far as to put mm in the report.

It is a pressure test, but the doctor would hold the distal or proximal phalanx in the performance of that test while compressing the wrist and waiting for numbness to occur. I would like to hear the rest of your sentence before saying for sure what you are hearing is Phalen's.
