First of . . .is two-tendon the correct way to type it or is it 2-tendon or is it either one of those just without the hyphen?

All I can say is thank heavens for QA when I get boogers like this guy. As much as I hate to add to their workload, I can't take a chance, either.

Diplopia of large bowel?

Report for laparoscopy. There were filmy adhesions from the posterior cul-de-sac to diplopia of large bowel and one adhesion from diplopia to the right ovary.
I have never heard of this before and cannot find another meaning for diplopia other than double vision. She repeats this 4 or 5 more times. HELP!!
