It is decubitus / decubiti

or

decubitus ulcer / decubitus ulcerS.

The S at the end indicates the PLURAL !

How would you then know if it is PLURAL OR SINGULAR????

"as well as" does not equal "and"
Subject: "as well as" does not equal "and"

i.e., the phrase isn't a conjunction and doesn't get treated the same way in a sentence. So your first choice is correct. I find that a lot of docs dictate w, x, y as well as z. I always change this to w, x and y as well as z.

Good catch.
