I searched Google books and found this - Osteoarthritis: Diagnosis and Medical/Surgical Management, page 355 - "clinically advanced OA demonstrates the "shoulder sign" with dorsoradial subluxation of the first metacarpal on the trapezium. Pain can be elicited by palpation of the dorsal radial aspect of the trapeziometacarpal joint, adduction of the thumb, or with rotation of the axially loaded thumb (grind test)
