A DS on an elderly man who is unable to be weaned from the ventilator.  Dictator states during physical exam that the patient was not (s/l) ''over worried on the ventilator''? ''over worked on the ventilator''?

I have been assigned some work to help the office "catch up" and this is not my normal assignment.  Can someone help me with ventilator terminology??  I may be far off, but here is what I think I am hearing.

__SIV m---__ rate set at 16 and tolerating the 550 and FI to 40 percent and a peak of 10.  ___longest set of peak___ratio of 182.  Her morning blood gas shows a pH of 7.48, a PC2 of 36 and a PAO2 of 73.

GENITOURINARY:  Bilaterally descended testis with a normal penis.  He has s/l [10 and 1].  s/l He was normal side to side during this exam.

underwent  "PSARP" ? two weeks ago.   Very clear, but have not been able to find. 

States colostomy is health and then states that the s/l "peroneum" is well healed........should that not be peritoneum?  Thanks.

Psych eval for a 7-year-old student with ADHD...

He says the following:

He showed a moderately delayed score in the working memory area when he had difficulty recalling S/L: wrote, auditorily presented information.  Weakness in this area may impact his ability to remember, recite, and manipulate sequences for academic tasks.

WROTE auditorily presented information?  TIA

Oropharynx is without erythma, s/l epistasis? , petechia, or s/l yeast? 

does this seem right?

Sounds like "blood pressure equal, good cap refill."  Would this be correct for a 2-year-old child's blood pressure?  Thanks so much!
