Some docs use apogee or perigee as point of reference in perineal or A/P repair (as in phases of moon, supposedly something to do with shape of perineal body like crescent moon, etc.) It's been years since I had that term in an op note.

We then paid attention to the perineoplasty. A triangular incision at the posterior s/l forcet was performed with the apex approximating 1 cm away from the anus. We then placed an s/l intergrafted stitch through the bulbocavernosis mucosa _____ tissue on the other side. An additional supporting stitch was then placed. At this point, we were able to reapproximate the vaginal and the posterior s/l forcet using a 2-0 chromic in a running fashion.
