trying to find medical words and phrases, much less cities and towns. If they cannot spell it, they don't get it. I won't waste my time looking. I am not getting paid for that kind of search...actually not getting paid for ANY kind of search. JMO

Her weight s/l "1 per 1980." Blood pressure controlled 130/80. Color is fair. Throat is s/l "cold." Thyroid tissue on palpation. Lungs clear.


 


I wish I could ask for help on the next sentence too, but I cannot make out one single word in it!!

Radiographs show s/l- a foot 80 degrees right thoracic, 76 lumbar.  ( it really sounds like he is saying foot, he is foreign, and I do not understand where foot would fit in, but Im not familar with typing these notes.

I have a name that I can't understand in the document anyway (person's last name) and I can't find it to verify spelling - so it's got to go to QA anyway, I come back here and post if/when QA gets it in case someoen else ever has it. TY for those that tried looking also.

Doc may have meant take 2 tablets in the morning and said the q just as a habit. I would certainly question having to take Lexapro at 2 a.m. In my opinion, don't guess - flag it.

It wasn't locked.  Sounded like lap or lapped.  Doctor was running words together and even made up a few of his own!  Gotta love it !

I have a doc who says stuff like that all the time...give a list of "this is normal, that is normal" only he says the same thing both times. I don't change it, I just flag it.
