I have never heard it before, but it does make sense. The patient was doing range of motion cold, and in your case, he is using the term "cold turkey", meaning the extremity he is trying to range is "cold", not yet warmed up. So, I would say yes it makes sense!!

I also believe the diagnosis of Disruptive Behavior Disorder Not Otherwise Specified should be ruled out for her given high levels of concerns of these types seen for her at school. However, it does seem possible that some of these behaviors are related to issues with attention span and/or learning, and in particular, auditory memory; conceivably, as these issues are successfully addressed, we may see less pronounced S/L: SUP-TUNE-IS on her part, at least in the classroom.

If it is ever in question, they can go back to the dictators original dictation (if available) and see that is what he/she said and they are directly responsible for the content of their reports.

I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN. Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading. Hope that helps!
