I think most databases would have it Last, First MD - sm

because when searching a patient, it would be searched as last name, then first:

Jones, James MD

If it is entered with MD after the last name, and I want to search for patient by the naem of James Jones, it will throw off the search.

Same for a name with Jr or Sr - I've seen most databases have names listed as:

Jones, James Sr

If listed as Jones Sr, James it alters the search quite a bit making Sr part of the last name, but having the Sr after the first name brings it right up.

Hope this makes sense. I'm curious too as to what others do.
