Unsure if this sounds correct or not.  She is speaking fast and not sure if I am missing it.

She has a history of cerebellar infarct, (s/l might have been the embolic) and she has been on a combination of Coumadin and Plavix

TIA 

The patient was out of the __________ time window as this was greater than 3 to 5 hours.  The patient was transferred by ambulance and was admitted for a possible stroke workup.

s/l TVA.  Would make sense if CVA, I guess, but really sounds like a "T" -- is there such a thing?

Discontinued his ??max in the hospital.  He has never had an episode of TIA or stroke in the past. 

I can only hear the max and just wondering if anyone could help with a medication for stroke or TIA that ends in max. Thanks!!

Doc is dictating patient is not a candidate for s/l altaplace or TPA therapy.

Diagnosis of acute right middle cerebral artery ischemic cerebral infarction and acute right internal carotid artery and right middle cerebral artery occlusion.

In exam, he says:

He has flaccid left hemiplegia of the left upper and lower extremities that does ______ (rep/rub) on the left on plantar response.

In view of the ___________ (rep/rub cuff/calf) it was felt that the patient would benefit from full anticoagulation for a short term.

Any help on this would be appreciated.  Thanks.

Stumped on a word - here is the sentence.

We discussed again the options of observation versus full open repair versus remote Referral Center for off-label investigational branch vessels _____ versus thoracoabdominal aortic paving with branch vessel reconstruction. 

This is something like femcraft, themcraft.  I have tried to google this but cannot come up with anything.

The difference in the modulus of elasticity between the metal and the bone can definitely lead to some  s/l furching and mild discomfort.  It has to do with a recent hip replacement.  Any ideas?

she is oxygen dependent on 3 liters with a convention.  I have tried lots of words, condension, contention, etc. and nothing seems to relate.
