We're not allowed to do that even with surgery. Plus, docs usually say, for instance, 2:30 position or at 2:30.

My company has an appeals process to put through for disagreement with QA. If you have no appeals process, would refer the report back to the QA supervisor or your direct supervisor for clarification.

I usually address it to:

Cardiology Associates

and then use the salutation, Dear Doctors:

Otherwise, if the dictator dictates each name you could do each name on a line, then the address and then either put the salutation as Dear Doctors, or Dear Drs. Smith, Jones, White and Green.

Dr. Sam Smith

Dr. Joe Jones

Dr. Willy White

Without the spaces, of course. :)

Subject: I really think it depends on the work...

I have worked from home and been paid both by the line and by the hour (as I am now).  Unfortunately, the company was transitioning to voice recognition with us being paid half the line rate.  The VR program had loads of bugs and I'd take as long, if not longer, to edit a report as it would for me to have simply typed it outright and my money went down drastically.  It wasn't worth it and I quit (there was literally a mass exodus of MTs from this company when this happened).

I now happily work by the hour doing both straight transcription, as well as QA and editing.

Prosthetic hip reduction. The term s/l "joo-DAY" views - ??? Thank you for any help.

Following reduction of the hip, single AP fluoroscopy view revealed that the femoral head sat eccentrically in the acetabulum, and this could simply be consistent with poly wear given the large number of dislocations she has sustained. Therefore, _____ (s/l joo-DAY) views, both internal and external oblique views of the right hip were obtained, which confirmed indeed the femoral head was seated in the cup and not anterior or posterior to the cup.

Subject: It is just the "old way" of spelling it, just like orthopaedics and orthopedics.

One such example is the American Academy of Orthopaedic Surgeons.  To an orthopod, this is the correct of spelling it and not "orthopedics" but both are accepted as correct by the general populace.  There are places that still use Paediatrics as opposed to Pediatrics, but they are still both right.  Just spell it like your QA wants.  Every account is different and every client has their own likes and dislikes.

True, it is in all what you are allowed or not
Subject: True, it is in all what you are allowed or not

allowed to do. However, at one of the first hospitals I worked for (back in the early 80s) for example, we had a doctor who was constantly using abbreviations which we were not allowed to do in diagnosis. I will NEVER forget this. His diagnosis was "P.M.S.O.B." The transcriptionist, of course, changed this in the diagnosis to read "Shortness of breath at night." He flew off the handle - this abbreviation meant "poor, miserable, son of a B****" and that is what he wanted in the chart. It was referred to the Hospital Director who told our department manager that even though this was definitely not correct, he dictated it and wanted it there and there was not much we could do about it if we wanted to keep our jobs. This is a lesson that taught me never to second guess a physician. Granted this was not at all politically correct (we didn't have that term back then), but that is what he wanted, so I don't change anything in that vein - I may question it and flag it for the higher ups, but I don't change it.

I am allowed to edit and correct
Subject: I am allowed to edit and correct

I am allowed to make changes to what he says, which is why I asked the question. I am allowed to edit and change what the doctor says and how he says it - and I do a lot of that in the course of the day because some of the things he says are just a mess. "Retarded" did seem a little derogatory to me and I think I will put it in better terms than that.

I guess it comes down to what the rules are that you work by - whether you can edit or if you are verbatim. If it was verbatim I wouldn't have questioned it (even thought I personally feel it is insulting) but since I have the option to change it, I was just wondering what the correct term was now because I know it keeps changing. Thanks for all the opinions!

Subject: Thanks...that option is not allowed as my account is verbatim...

I put the word in as dictated with quotes around it and then sent it off to QA. I noticed later that they just deleted it and put a blank and sent it through without the word. Yet one more reason why I hate being on verbatim accounts!
