for one of my current radiologists. He says "strandy" plain as day, so that's what I put. Grates on my nerves, but he's the radiologist.

2. Left carpal tunnel syndrome.

Will get EMGs. The patient has evidence of s/l palmarthrorheumatosis. He runs it together, so it may be palm (or palmar) arthrorheumatosis.

Any help is appreciated. :)

If you are transcribing, the correct way is:

V code: Parent-child conflict.

Unless you are a certified coder, it would be wise to avoid inserting the actual code into the transcription as the MD/psychologist did not dictate it. The codes can change from year to year and you could enter the wrong code depending on the date of the service and which coding resource you are using.

I am new and have an ophthalmology account. I am not having much trouble with the terms, but I'm unsure how to punctuate and/or format the lens prescriptions.  Does anyone know of a good website that explains this or provides examples?  Thanks!

to patient as "sister" throughout report, is "sister" capitalized or not?  Neither way looks correct to me.

I've come across a dictation that is talking about NIDDM.  It sounds like he's saying "he was initially of sulfanuria and switched insulin in Puerto Rico".  The other part is "He has never been on metformin or glybizone."  Any suggestions??

Just started OB/GYN myself. If you don't have this:
Get the book "Stedman's OB/GYN Words."  I got mine at Barnes & Noble "used and out of print" for $9.00 and it's the newest one out.  Perfect condition when I got it.  It's getting a bit "dogeared" already.  I use it constantly when doing OB/GYN, as I've never done it before.

we MTs had very few references, certainly no Google. We would beg doctors to give us their old PDR (Physicians Desk Reference) and would find lots of our S/L there. We oldsters were definitely word detectives in our day.

as *nn* as I believe I was the first to comment on this kathie's suggestion for caudal vater syndrome as having nothing to do with the low back. Anyway, then she made the comment about all diabetics having high blood pressure, and it raised the hair on the back of my neck. When I read LOL-sm post, I literally busted up laughing! I thought it was so funny and so FITTING. it put an end to the thread and made every one laugh...good ending to something that was getting ugly and sort of put the QA back a step or two, as well! It was great! Still laughing!

I figured I was spending way too much time trying to decipher his jumbled mumbled dictation. He can either learn to speak correctly and coherently or spend his time trying to figure out what he was trying to say to fill in the blanks!!!

You can kind of see how a hair would be so fine, and babies have that sweet baby plumpness so that it would be hard to see the hair until the baby had redness and irritation, but does the hair just sort of get worked into the area from a sock or something that was in the dryer and had a hair already on it? Who knew there were so many things to worry about??

Normally they are plain films.  However, sometimes they are actually talking about the plane or surface orientation of the exam so you have to be careful.
