Ehrlichia titer, I would imagine. The organism is Ehrlichia. Those beginning sounds can throw us sometimes as in ..."bovicea" titer

  1. Schizoaffective disorder.

  2. Peptic ulcer disease.

but the doc I have is saying ill-descript chest pain and ill-descript complaints.    I have heard them say nondescript and I cannot even find ill descript in my dictionary.  Is there such a word? or am I spelling descript wrong??

When you get stumped like this try...

...asking yourself a few questions that can help.

For example: You're looking at the cervix (visualize with me), there isn't much in the way of structure in the cervix, so your choices are limited. You've got a clue in the dictation--orifice. What is the orifice called? "Os." Sounds like you've got a doc that belches his way through the term, but the only logical choice for this particular term would be os.

Isn't transcription fun?!

If that measurement is correct, 0.45 mm, it sounds to me like the diameter of a WIRE not a plate...without more info, I can't think of anything that would fit here. I suppose 0.45 mm could be the amount of DISTRACTION created if they're using a plate and screws, but again without more info, i don't know...sorry!

Ortho doc is dictating impressions and says:  Mild bursitis and tendonitis of the right shoulder ???? ieou ?? syndrome???


He actually says a - e - i - o -u  and then what sounds like syndrome. 


Am I hearing things?

She is noted to have a 3cm red mass just posterior to the arytenoid cartilage at the esophageal inlet  When she swallows this mass disappears into the esophageal inlet and then about 5 seconds later it protrudes back up behind the arytenoid cartilages.  It gives the appearance of an air filled esophageal mucosal sac.  


 


The patient brought with her a lateral x-ray of the neck which shows the s/l prevacubital space is very widened in the retrolaryngeal region. 


 


Any suggestions?

Moderate ischemic cardiomyopathy. There is no clinical sign of congestive heart failure (CHF); however, on the pacemaker interrogation, s/l optigal or optimal trend is turning outward. Her weight is also the same as last time. Pacemaker interrogation confirmed the patient indeed had recurrent nonsustained VT and appropriate intrinsic VT not due to pacemaker. The (same s/l) trend was also increasing over the threshold.

I hear 3 syllables. Have been trying to get this for about an hour and can't. Any ideas?

Thanks - but this one has me stumped.  It sounds pretty clear.  Only other thing it could be would be "see MR card" but that does not really make sense either?!  Guess I will have to blank it.
